We are seeking a compassionate and dedicated Licensed Social Worker to join our hospice care team in Central Texas. This role offers flexible weeknight and weekend on-call hours within a highly supportive, family-owned organization.
Our team values long-term collaboration, with an average tenure of over 5 years among Social Workers and 8 years among RNs, reflecting our commitment to compassion, professionalism, and patient-centered care.
Key Responsibilities
- • Provide emotional and psychosocial support to hospice patients and families
- • Participate in flexible on-call coverage during evenings and weekends
- • Collaborate with interdisciplinary hospice care teams
- • Assist patients and families with care planning and available resources
- • Maintain documentation in compliance with hospice regulations
Candidate Requirements
- • Active Licensed Social Worker credential
- • Hospice experience preferred
- • Strong commitment to compassionate, patient-focused care
- • Ability to work flexible on-call schedules
- • Willingness to relocate to Bryan, TX before starting
