We are seeking a detail-oriented Home Health Clerk / Administrator to provide essential administrative support for home health operations. This role is responsible for managing patient records, scheduling services, processing intake and discharge documentation, and coordinating communication between patients, families, and medical staff.
Key Responsibilities
- Manage patient admissions, discharges, and transfers in compliance with HIPAA and company policies
- Maintain and update patient records including medical history, care plans, and daily logs
- Coordinate scheduling for nurses, therapists, and other healthcare staff
- Handle inbound and outbound calls related to patient care and appointments
- Process intake documentation and discharge paperwork accurately and on time
Documentation & Data Management
- Update and manage Electronic Medical Records (EMR)
- Verify insurance information and assist with medical billing documentation
- Transcribe physician orders into electronic systems accurately
- Ensure completeness and accuracy of all patient-related documentation
Communication & Coordination
- Serve as the primary point of contact for patients and families
- Provide information regarding services, scheduling, and care processes
- Coordinate communication between clinical staff, patients, and families
